How We Store Your Information

Assessment and eligibility-related documentation is stored within our agency's electronic document management system. This system allows us to retain files efficiently. Only DSO staff have access to this information. This system complements an agency database that allows us to manage registry (or “waiting list”) information.

Data from the Application form and the Supports Intensity Scale® is entered into the “Developmental Services Consolidated Information System” (DSCIS.) This is a provincial database used by DSO’s to record and hold application data. The Ontario Ministry of Community and Social Services can access non-identifying information for planning purposes. For example, they can identify how many people want residential, caregiver respite, or other types of supports. No other agencies can access this information without your permission.
