Person-Directed Planning

Person Directed Planning can help a person and their support network develop a “personal life plan.” This can include developing a personal vision, and identifying different dreams and goals that a person would like to achieve. Person-directed planning can involve a person’s family, friends, support staff and other people who are important to them. A Ministry-funded agency can help people with this.

Passport:

Passport is a program that helps adults with a developmental disability to participate in their communities, and helps caregivers take a break from their caregiving responsibilities.

Passport/Direct Funding allows someone to get help to develop a person-directed plan that builds on their strengths and interests, and identifies the supports needed to achieve their goals. People can use up to 10% of their total Passport funding for these expenses.

For more information on person-directed planning, please check out the Ministry of Community and Social Services website.
