The Application Form

Once we have confirmed that a person is eligible for Ministry-funded services and supports, we can begin to complete the provincial application package. To do this we need at least two people, called respondents, who can speak knowledgeably about the person's support needs in a variety of areas.

The first meeting is with an Assessor/Service Navigator and the Application for Developmental Services and Supports (ADSS) is completed. This meeting is all about getting to know you in these areas:

  • Communication
  • Current living stuation
  • Likes and dislikes
  • Dreams and goals
  • What is important to and for the person
  • Current supports and supports being requested
  • Services you are seeking

At the meeting the assessor will also gather information about the following:

  • Medical conditions
  • Behavioural needs
  • Financial status
  • Caregiver concerns
