What Happens When You Call

If someone would like to apply for services and supports – or just wants more information – they can call our office toll-free at 1-888-941-1121. A receptionist will ask what the person is seeking, where they live, and then connect them with a DSO Coordinator in their community.

A DSO Coordinator will explain the process and documents required for eligibility. Information about developmental supports and services will be given.

The Coordinator will complete the intake referral if the person consents. Demographic information will be asked, such as, age, date of birth, and address. The Coordinator will also ask about current services and services that are being applied for.
